To get started with The Caring Group, begin by completing the Request Evaluation form below. We’ll guide you through each step from there.
Step 1: Request an Evaluation
Fill out this form to get started. We’ll gather a few details about your child and your insurance.
Step 2: Prescription from Your Provider
We will need a prescription from your child’s primary care provider on file. This can be faxed to our office at 618-607-0042. If your insurance requires authorization, we will guide you through that process.
Step 3: Insurance Review
Our team will contact you to review your benefits and discuss any potential out of pocket costs.
Step 4: Scheduling
Your child will be matched with a therapist, who will reach out to schedule your evaluation and first visit. Availability may vary.
Step 5: Intake Form
Before your first visit, please complete the intake form using the button below. If you prefer, you may download and bring completed forms to your appointment.
Additional Forms (If Needed)
Depending on your child’s needs, your therapist may ask you to complete a few additional forms. These can be found below and will be discussed with you if needed.
